This little girl came to FIRST Ortho when she was 10 with several baby teeth in her mouth, with the issue of crowding and a bite problem. X-rays showed our concern with her canine teeth impacted in the palate. Teeth left in this position are an issue as they can damage the front teeth causing root resorption and are more difficult to align when older.

At age 10 she had impacted canine teeth. These teeth would have remained in the palate unless we helped. The crossbite gave an asymmetrical facial appearance and caused extra wear of her teeth.

After a thorough assessment, Dr Patel chose the best treatment: By timely extractions of her baby teeth, it allowed the canine teeth to erupt over a period of years without the need for oral surgery. Left alone surgical intervention is required.

At the age of 15, we reviewed the treatment bearing of mind extreme cases of asymmetry and crossbites, can result in surgical treatment to align the teeth and jaws. We opted to start orthodontics with an expander and standard steel braces.

Facial and dental asymmetry is one of the most difficult treatments if one is trying to avoid a surgical option. The client underwent a long treatment plan of 31 months. This is quite a severe case resolved to a good level with orthodontics only – without surgical intervention at any stage.

Retention is critical on a long-term basis. The end pictures are two years post treatment.

Memorable moments: All the fine detailing to set the bite.

The parents and the child were delighted with the outcome considering we avoided surgery for the canines and the jaws. These are the challenging presentations we specialise in.

This is minimalist perfection. With just 1 month of orthodontics with a simple brace and NO FURTHER treatment we corrected a reverse bite (underbite), which caused her jaw to sift to the side, and allowed the lower gums to improve with recession.

She returned 7 years later and her second teeth, although not mathematically perfect, are ‘perfectly acceptable’ both aesthetically and cosmetically.

Most Memorable moment: Meeting her 7 years later.

At the age of 33, prior to seeing us, this delightful lady received advice that her reverse bite could only be corrected through jaw surgery due to the asymmetry in her lower jaw. From a mathematical standpoint, this approach seems straightforward, directly addressing the root cause of the issue. However, it comes with a radical and high-risk element. A critical question arises: Why alter the entire face if the concern is solely focused on the teeth?

Our philosophy advocates for enhancing facial aesthetics within the established parameters. This introduces a constraint to the treatment, demanding an additional skill set. When successful, navigating these constraints becomes one of the most gratifying aspects of tackling orthodontic challenges.

What was our plan? We mutually agreed on a treatment course involving a quadhelix to widen the upper arch and the application of fixed braces. Remarkably, within just 9 months, the treatment was concluded in 2015 and has since remained stable.

In cases of reverse bite, timing is crucial because growth can make the situation worse and lead to additional issues. The reverse bite was fixed in two to three months, but the therapy took an additional 10 months because an impacted tooth broke through rotated into the palate.

Nevertheless, the transformation of the facial structure is astounding. In his adolescent years, some small remedial therapy is expected for a perfect result.

Here is an illustration of how to correct a backward bite caused by an undeveloped upper jaw using an expanded upper arch and a reverse headgear/facial frame.

Early treatment in the young is crucial because reverse bites worsen with growing and develop more negatively. Early intervention can reduce the need for future surgery or more complicated medical care.

In more complicated cases like this one, it won’t completely remove the need for more treatment in the future.

The key to this excellent result was the overbite and canine position correction.
