This is minimalist perfection. With just 1 month of orthodontics with a simple brace and NO FURTHER treatment we corrected a reverse bite (underbite), which caused her jaw to sift to the side, and allowed the lower gums to improve with recession.

She returned 7 years later and her second teeth, although not mathematically perfect, are ‘perfectly acceptable’ both aesthetically and cosmetically.

Moderate to marked protrusions in children deserve attention for a few reasons:

  1. The aesthetics and possible affect on their development of confidence or self esteem.
  2. Teeth are more prone to damage due to trauma and follow complication with the management.
  3. Intercepted at the right stage, the changes are rapid and remarkable. This window of opportunity gets smaller with increasing age and can be markedly difficult age 15+.
  4. Some may add affect on mouth breathing, facial development, nasal blockages, sleep disturbance, mental development etc etc. We take a more practical view as alarmist overtones is not a good start off point when it is not backed up by evidence based research.

The orthodontics was completed in 12 months and was stable into her teens when permanent dentition treatment is considered.

Most Memorable moment/s: Her lovely smile on meeting at the first visit and meeting her father.

A gap between the upper and lower teeth, as illustrated here, is often associated with thumb sucking. Despite the presence of typical indicators, there was no relevant history in this particular case.

To address the issue, we agreed to use a tongue guard, facilitating the natural movement of the teeth to close the gap at the front of the mouth (openbite) within a span of 7 months, all without the need for an external brace. The absence of closure would have suggested that the openbite was rooted in the jaw structure, presenting a more complex challenge.

To refine the treatment and enhance stability and perfection, we applied a metal brace. Bonded retainers were then affixed to preserve the progress and solidify the outcome.

The concluding black and white photograph was taken at our Gants Hill studio, capturing the poised and self-assured demeanour of a 12-year-old. The maturity displayed in the pose renders this image one of my favourites, captured right at the end of a one-hour photoshoot.

Protrusion of the upper teeth, makes then 6x more vulnerable to damage. Correction of protrusion is both aesthetically and functionally beneficial.

However, in the young, the baby teeth can be too loose to support a functional brace. In this case fixed brace can be a solution though the design features of these braces are unique and requires greater skill.

In cases of reverse bite, timing is crucial because growth can make the situation worse and lead to additional issues. The reverse bite was fixed in two to three months, but the therapy took an additional 10 months because an impacted tooth broke through rotated into the palate.

Nevertheless, the transformation of the facial structure is astounding. In his adolescent years, some small remedial therapy is expected for a perfect result.

Here is an illustration of how to correct a backward bite caused by an undeveloped upper jaw using an expanded upper arch and a reverse headgear/facial frame.

Early treatment in the young is crucial because reverse bites worsen with growing and develop more negatively. Early intervention can reduce the need for future surgery or more complicated medical care.

In more complicated cases like this one, it won’t completely remove the need for more treatment in the future.

Is this orthodontics at it’s finest?

Numerous unfavourable characteristics, including an undersized and undeveloped lower jaw, extreme crowding, upper teeth sticking out and impacted upper canines on both sides.

Treatment with functional and fixed steel braces. A fantastic patient since a young age who handled orthodontics with ease.

A striking dental and facial alteration.

This is orthodontics at the finest.
